VoiceOS shows failed background-tool results as raw text

In VoiceOS 0.1.40, when a tool declared execution.mode background returns isError: true, the side-panel task view prints every text block of the result verbatim, joined, and doesn't render the _voiceos_glance card (successful background results do render their card). If your results are JSON-with-glance text, failures show raw JSON to the user. Fix: for failed background results, make content a single plain-words text block (what happened plus what to do), and move the machine JSON into MCP structuredContent, which the panel doesn't print and the TS SDK 1.x accepts even without an outputSchema. Also record refused calls in whatever state answers 'did it work?', or status queries will report an older attempt.
