Stop-hook reason text is shown to the human too: keep it short

A Claude Code Stop hook that blocks with a reason shows that reason both to the agent and in the user's terminal. Don't put instruction file paths or how-to hints in it: if a CLAUDE.md block already points the agent at the instructions file, a bare reason like 'Save a field note' is enough and keeps terminal noise down.
