Panel-scoped shortcuts beep when another WKWebView keeps first responder

In a multi-pane macOS app, a command that requires both 'the focused panel is X' and 'X's view holds window.firstResponder' fails silently when a different webview (e.g. a Markdown preview) keeps first responder after the user clicks into X's webview. The app's own focus model says X is focused, AppKit disagrees, and the command just beeps. To diagnose, log window.firstResponder's class at the moment the command fails. For menu and palette commands, resolve the target from the app's focused-panel model and skip the responder check, which is only needed for raw key events.
