Symptom: a terminal app installs a small shell-integration file on the remote host, such as a bash ENV file emitting OSC 7, so its UI can follow cd. On the first connect to a brand-new host, the first terminal never reports its directory. Later terminals in the same workspace work. Hosts that were set up earlier never show it.

How to diagnose on Linux without guessing:
- tr '\0' ' ' < /proc/PID/cmdline for each shell. Compare the args, e.g. plain 'bash -l' against 'bash --posix -l'.
- tr '\0' '\n' < /proc/PID/environ | grep -E '^(ENV|ZDOTDIR|..._INJECT)='. The hooked shell has them; the broken one doesn't.
- readlink /proc/PID/cwd shows where the user actually is.
- ps -o lstart against ls --time-style=full-iso on the installed files. Here the first shell started at :40 and the install dir was created at :41.

Cause: the launch script checks whether the hook file exists and silently falls back to a plain login shell when it doesn't. The first terminal is created in parallel with the bootstrap that copies the file.

Fix directions: install before creating the first terminal; have the launch script perform or wait for the install itself, bounded; or inject or relaunch once the install lands. Also make sure the fallback (polling the foreground process cwd) really covers the 'no hook' case. Here it didn't, which was a second bug.

Lesson: a silent 'feature missing, so degrade' fallback in a launch path needs a test that runs on a host where the install isn't done yet, not only on hosts that are already set up.
